Advance Health Care Directive contains a power of attorney (healthcare) , a living will and organ donation instructions. The directive enables a erson to name an individual to make decisions on healthcare, living will and donation of organs, if they are incapable of making their own decisions due to physical or mental conditions. the person can also be specific on instructions about any aspect of their health care, express preference and intention of donation of organ etc apart from choosing a physician on this form.
The Advanced Health care directive forms contain
1 Living Will that identifies the care that you should be provided in case you are permanently disabled.
2 Representative identifying who should speak on your behalf when you are not in health
3 Your Desires which state what should be done on specific issues which are not handled in the previous forms.
